Lunar.js is pretty straight-forward to use.

Give it a date string and it will return:
0 for new moon
1 for waxing crescent
2 for first quarter
3 for waxing gibbous
4 for full moon
5 for waning gibbous
6 for third quarter
7 for waning crescent
