Form Maker was developed so site admins can easily build Web forms from an easy to use graphical interface.

The commercial license of this plugin will allow administrators to build Web forms with more than five fields.

Installation:

Unpack in your modules folder (usually '/sites/all/modules/')
Enable under Administer > Site Building > Modules
